How much are you able to change the hardware? can you remove all the disks except the boot disks, "devfsadm -Cv" and check to see if the fault still occurs? assuming that you're using ZFS is the "bad" disk appearing in the "zpool status"? is it appearing in an "iostat -n" command? Jon
