You need to upgrade to snv_134 first - the direct jump from 2009.06
(snv_111) is problematic.

Try this:

$ pfexec pkg set-publisher -O http://pkg.openindiana.org/legacy
opensolaris.org
$ pfexec pkg image-update

That should take you to snv_134, using the OpenIndiana snapshots of the
OpenSolaris dev repository.

Once that completes, reboot into the new boot environment, then try the
upgrade to oi_147 again.

I'm writing up some more involved instructions but VirtualBox is being
slow (well, actually I'm thrashing my machine, but it's easier to blame
the virtualisation ;-)
