Don't know for sure, but I think some sites do that by having a lookup table for ranges of IP addresses associated with various countries - the server obviously knows the client's IP address, to communicate with it.

Of course, given VPNs and remote points-of-presence (not to mention TOR, probably), that's hardly bulletproof.
